If you are considering facial rejuvenation surgery, there are a multitude of various procedures that can give you a more youthful appearance. The Gold Standard of facial rejuvenation surgery is the facelift also known as the cervicofacial rhytidectomy. This procedure has been utilized by not only the Hollywood elite, but by every facet of the American mainstream to help people attain the look that reflects the age that they feel.

Inevitably and as we all start to age, the areas that typically start to show our age first appear around the eyes, the mouth and the neck. The lateral 1/3 of the eyebrow will start to descend, and instead of having a nice arch with the lateral corner of the brow higher than the middle, it will eventually become more flattened, then descend lower than the middle. The upper eyelids will slowly start to get heavier and wrinkles will begin to appear around the corners of the eyes, also known as "crows feet". The lines between the nose and the mouth will begin to deepen, also known as the nasolabial folds, and lines at the corners of the mouth may appear, also known as marionette lines. The jawline may start to show signs of aging when the skin begins to loosen, and jowles will appear. The neck will lose its elasticity, and will become loose and wrinkled. Face and neck lift procedures address most of these changes, but ancillary procedures also must be performed to keep the facial features in harmony. When having facial rejuvenation surgery, the procedures that typically are performed in conjunction with the face and neck lift are blepharoplasty or eyelid lift, browlift or forehead plasty, chin & cheek enhancement and laser resurfacing or dermabrasion. When performing these surgeries at the same time, it allows the patient to experience one recovery, and a balanced rejuvenated look.

Facelift surgery has been performed successfully for decades, and a number of different techniques exist. Some patients are better suited for one procedure over another, and are usually evaluated in depth prior to their surgeon deciding which procedure to perform. The skin facelift is a procedure where incisions are made around the ears and sometimes extending into the hairline in front or behind the ears or both. After the incisions are made, the skin is lifted off of the soft tissues, and pulled back to tighten the skin. This procedure can give some improvements, but is typically short lived and thus not typically performed on most patients.

The superficial musculoaponeurotic system (SMAS) rhytidectomy addressed the soft tissue layer that covers, surrounds and attaches to the deeper tissues and structures of the face and neck, including the entire cheek area. This layer also attaches to the platysma muscle; this is the muscle that covers the lower face and neck. By addressing these tissues, the jowls, cheeks and neck are tightened as a unit, and the skin is gently re-draped over the facial architecture. The deep-plane facelift, which elevates the tissues on a level surrounding the bones of the face, incorporates the endoscopic approach to address the face, midface, forehead and neck and tightens the soft tissues is often performed simultaneously with skin resurfacing, chin and cheek implants and eyelid surgery. There are other variations of this procedure such as the FAME facelift, also known as the finger assisted malar elevation facelift where digital maneuver is performed in a relatively avascular plane, over the muscles and underneath the malar fat pad to assist in separating the tissues prior to tightening them.

Once you have been evaluated for your surgery and the decision has been made exactly what procedures you will be having, you and your surgeon will choose a surgery date, then you will be scheduled for your pre-operative appointment. Depending on your age, your health status and the medications you take, you may need a pre-operative clearance from your primary care doctor, and EKG and possibly other ancillary procedures. At your pre-operative appointment, your photos will be taken and your instructions will be given to you. You will have the option to stay at a post operative recovery center and your prescriptions will be given to you for you to get filled prior to your surgery day. The surgery is scheduled at an accredited facility where the safest environment is offered for the comfort and safety of the patient. You will meet with your anesthesiologist prior to surgery who will stay with you during the entire procedure where you will be monitored throughout the case. When you awake, you will be in minimal to no discomfort, and you will recover comfortably until you are ready to be discharged. You will likely have a turbin type dressing around your head and ears, and may have an elastic strap around your chin to help minimize swelling.

Typically, you will return the following morning for your first post operative visit, where your dressings will be removed and Dr. Bandy will evaluate you. Typically on the second or third post operative day, you are able to shower and wash your hair. If you have had eyelid or forehead procedures, your stitches will be removed on the 4th or 5th post operative day. All other procedures will be evaluated according to your recovery. Most people are healed enough by the first week to return to work or their regular social activities and look fantastic. Returning to exercise and strenuous activities usually requires a few weeks before you are given clearance by Dr. Bandy. Many patients feel that their decision to have facial rejuvenation surgery is the best decision that they have made in looking and feeling younger and more vibrant.

When looking to have facial rejuvenation, patients will first go to see plastic surgeons requesting information about face lift. The primary areas that start to age on the face are around the jaw line. This includes the jowls, the neck and the eyes. There are multiple types of facelifts depending on 1) the structure of the tissues; 2) the patients bone structure; 3) The density of the skin and 4) the amount of previous sun exposure if a person is looking to get improvement of primarily skin problems such as wrinkles or sunspots. Oftentimes a facelift is not the ideal procedure and the patient can achieve excellent results with laser resurfacing procedures. However, if there is looseness or laxity along the jawline in the nasolabial fold areas in the cheeks and in the neck , then the ideal procedure would be a face and neck left. There are a multitude of surgical approaches for this surgery and they will vary with the looseness of the skin. The skin facelift is helpful for the younger ages in the mid 30s to early 40s who seek a facelift because of early facial skin laxity. Often times in this age group people will show signs of solar elastosis resulting in laxity or looseness of the skin but still good integrity of the deeper tissue. In the mid 40?s and older age group, the incision sites for the surgical approach as well as the depth of the tissue dissection will vary depending on several factors. One type of deep plane facelift deals with the tissue surrounding the muscles in the face. By elevating these tissues, patients can get excellent results with this procedure. A good candidate, for a first- time that they are having their facelift surgery, can get excellent improvement and can be a very good surgical candidate is a vertical fashion facelift which will probe the jowls and jawline by elevating the deep investing. The skin is then re-draped in a slightly vertical and horizontal backward directions to increase skin smoothness and skin tightening. This procedure is often done in women and men in their 40s through 60s. However even patients in his or her 70s and 80s can have this procedure and get excellent results. However, oftentimes they need to do additional ancillary procedures to get the best results such as laser resurfacing. Typically, patients are best served with having their eyelids, brow, and facelift, as well as their skin done at or around the same time so that the tissues are in harmony.

"Once the incision is made there are a variety of types of facelift procedures and the type of procedure chosen is typically adjusted to what each individual patient's requirements are," says Dr. Amy Bandy. Further, Dr. Bandy says that "a straight skin face and neck lift is performed by elevating the skin and subcutaneous tissue off of the facial features and gently pulled and excess skin is removed, and suture the face along the incision line."

Although a widely recognized procedure, this is not a commonly performed procedure says Dr. Amy Bandy. "However, it can be done in a more youthful face that has more skin elasticity.

Another type of facelift procedure is called the smas facelift (sub muscular aponeurotic system) through this type of procedure, after the skin has been elevated about a third of the way, anterior on the face and neck, then the muscular aponeurotic tissues are elevated off of the facial musculature and then retracted posterially and superially and trimmed and set into place.

"Doing so also tightens the jaw line in the neck, repositioning the platismal muscle in the neck. after that has been performed, then the skin is gently pulled and redraped, trimmed and sewn into place," says Dr. Amy Bandy.

Dr. Amy Bandy goes on to say that "another type of facelift procedure is called a smasectomy; after elevating the skin, the smas tissues are graphed and trimmed and sutures are placed to re-approximate the remaining smas tissues, and this can give a very similar look as the full smas facelift."

It does not end there. Dr. Amy Bandy describes that "there is also a facelift that's called a deep plain facelift, where through an endoscopic method where small incisions are made around the temple area and around the ear, the tissues are elevated up off of the bone itself in a subperiostial fashion meaning behind the muscle and directly onto the bone." "Then those tissues are advanced and moved into a more youthful position. with this procedure, the downtime and recovery is extended and there is longer postoperative swelling because of the deep tissue or deep plane dissection," says Dr. Amy T. Bandy.

Dr. Amy Bandy says that most facelifts, whether they are subcutaneous smas, or smasmectomy facelift, will require approximately about 7 to 10 days post-op recovery prior returning to normal activities in situations where patients want to be fully healed to where they do not look like they just recently had surgery. "Any sutures or staples are removed at approximately the third day post operative day, and the patient can typically get in the shower and wash their hair in as little as two days after their surgery," said Dr. Amy Bandy.

Lastly, Dr. Bandy says that "dressings are removed after the post operative day so the patient can go about normal daily functions if they are not concerned about having obvious signs that they've had surgery, because the first week they will have some bruising and swelling and this is the tell tale signs of having had surgery."
